Important Scheduling Policies

Please review these policies before scheduling:

• You will receive a required intake questionnaire via email after booking your consultation.

• The questionnaire must be completed at least 72 hours before your appointment.

• Failure to complete the questionnaire by the deadline will result in cancellation of your appointment. Consultation fees (if applicable) are non-refundable.

• Missed or canceled appointments may be released to another veteran on our waitlist.

• No exceptions will be made to these scheduling or intake requirements.

These policies allow us to maximize our limited appointment availability and ensure every veteran receives the attention they deserve.
